Announcing the winner in the category of Primary Care Partnership of the Year is Lucy Glynn, community manager at X-on Health.

Winner: Vertis Health, Commissioned by NHS Herefordshire & Worcestershire and Worcestershire County Council

Congratulations to the teams at Vertis Health and NHS Herefordshire & Worcestershire and Worcestershire County Council.

This partnership provided vital health services through a collaboration with PCNs, secondary care, public health, and VCSE partners, improving access, early diagnosis, and prevention. It saw two vans deliver services up to six days per week, including NHS Health Checks, support with GP registrations, CVD/lipid and HbA1C testing, and signposting to a wide range of local services.
